This is the standard form for any mod application:

What is your Minecraft Username?

How long have you been playing?

How long have you been a member of WweeeCraft?

How involved are you in WweeeCraft?

What makes you want to be a Moderator?

Do you have any current Moderators who would recommend you?

Any other qualifications:

This must be filled out accordingly to even be considered for a Moderator Postition
